1. Program Overview
The Smile Restoration Fund (SRF) is a program designed to provide financial assistance for cosmetic dental procedures to eligible individuals. These rules and regulations govern the application process, eligibility criteria, and award distribution.
2. Eligibility Requirements
To be eligible for the Smile Restoration Fund, applicants must:
- Be at least 18 years of age
- Complete an oral health evaluation with a participating dental provider
- Have completed all necessary basic dental work prior to cosmetic procedures
- Be a suitable candidate for the requested cosmetic dental procedure(s)
3. Application Process
The application process consists of the following steps:
- Submit an online application through the official SRF website
- Complete an oral health evaluation with a participating dental provider
- Receive a treatment plan and cost estimate from the dental provider
- Complete any required basic dental work at your own expense
- Receive notification of funding decision
4. Covered Procedures
The Smile Restoration Fund provides financial assistance for cosmetic dental procedures, including but not limited to:
- Dental implants
- All-on-Four and All-on-Six implant procedures
- Porcelain veneers
- Cosmetic dentures
- Clear aligners for cosmetic purposes
- Comprehensive smile makeovers
Note: Basic dental work such as cleanings, fillings, extractions, and root canals are NOT covered by the SRF and must be completed at the applicant's expense before cosmetic work can begin.
5. Award Determination
Award amounts are determined based on:
- The type and extent of the cosmetic procedure(s) needed
- Available funding at the time of application
- The recommendation of the participating dental provider
Award amounts typically range from 20% to 60% of the total cost of the cosmetic procedure(s).
6. Fund Distribution
Funds are distributed directly to the participating dental provider as a credit toward the applicant's treatment plan. No funds are distributed directly to applicants.
7. Applicant Responsibilities
Applicants are responsible for:
- Providing accurate and complete information in their application
- Completing all necessary basic dental work at their own expense
- Paying the remaining portion of the treatment cost not covered by the SRF award
- Following all pre- and post-procedure care instructions
- Attending all scheduled appointments with the dental provider
8. Program Limitations
The Smile Restoration Fund:
- Does not guarantee approval of all applications
- Reserves the right to deny funding for any reason
- May limit the number of awards based on available funding
- Does not cover procedures that have already been completed
9. Changes to Rules and Regulations
The Smile Restoration Fund reserves the right to modify these rules and regulations at any time. Changes will be posted on the official website.
